This CD was released by Wind-Up Records in 2004. The number is: 60150-13063-2. The case has some shelf wear and some scratches and has a crack and missing piece in the upper left corner. The CD has been tested and works fine. Includes insert. Please see pictures. A great gift for any Evanescence fan or collector!
